Patients may need a higher level of care or more intensive programming. We hope to be a stop in the journey. Our aim is to make the process smooth and rewarding for both you and your patient.

The initial referral

We are ready for a consultation with providers before making a referral to ensure our program is the right fit. Consultations can be done over the phone, our HIPAA-compliant video platform, or email to best suit your needs.

When you refer your patients to our care, we’ll facilitate a comprehensive initial assessment and offer recommendations or alternatives to both you and your patient. Following the initial assessment, our team will connect with you to go through our recommendations, obtain any additional clinical information, and to answer any questions you may have.

Partner with us

You have an established rapport and have nurtured a relationship. At the Rose program, we know your importance in the process. With consent, we stay connected via updates, summaries of care, and through collaboration.

Transtion back to you

We have strict standards when it comes to continuity of care. This is especially true when it comes to maintaining trust. For a seamless transition, you can expect a discharge summary, further clinical recommendations from our time with your patient, and the hope for feedback from you.

PUBLIC NOTICE

 

The Joint Commission conducts accreditation surveys of MCTS facilities on an unannounced basis. The purpose of the survey is to evaluate the organization’s compliance with nationally established Joint Commission standards. The survey results are used to determine whether, and the conditions under which, accreditation should be awarded to the organization. Joint Commission standards deal with organization quality, safety-of-care issues, and the safety of the environment in which care is provided. 

 

Anyone believing that they have pertinent and valid information about such matters is encouraged to contact administration. If the concerns in question cannot be resolved at this level, please contact the Joint Commission field representative. Information presented will be carefully evaluated for relevance to the accreditation process. Information about such matters must be made in writing and must also indicate the nature of the concerns.
